RUSH University Medical Center, a nationally recognized clinical and academic institution and the teaching hospital for RUSH Medical and Nursing Colleges, located in downtown Chicago, invites applications for the position of Advanced Practice Provider for Family Connects Chicago at RUSH. This role will function as a 0.50 FTE clinical provider Monday-Friday with the Family Connects Chicago program at RUSH.


Family Connects Chicago at RUSH is an in-home nurse service available to Chicago families with newborns. The service is simple: A registered nurse visits your home around 3 weeks after birth to check on mom, baby and the whole family. Included is maternal and infant health assessment and linkage to community resources to support newborn care.


The Department of Academic Practice Nursing (DAPN) is a department within the College of Nursing. We share a vision to create an equitable health system for all by empowering nurses and interdisciplinary teams to provide person-centered, collaborative care in partnership with communities, and to translate nursing innovation into policy advocacy and academic practice scholarship. Our mission is to exemplify academic practice where individuals and communities receive excellent health care from nurse faculty leaders, interdisciplinary teams, and students who participate in innovative care delivery and education practices that provide opportunities for professional growth, scholarship dissemination, and advocacy.


Duties & Responsibilities:

  • Completes and documents an accurate history and physical examination of patients.
  • Conducts comprehensive or episodic health assessments and develops patient care plans.
  • Due to this being a grant-funded role, your length of employment is contingent upon and tied to the duration of the grant. Your employment will be terminated in the event that funding does not come to fruition or at the expiration of the grant, whichever occurs first.


*Applicants with previous experience in community-based care with underserved populations and/or adolescent and child health care preferred.


The Family Connects Chicago Psychological Mental Health Nurse Practitioner (PMHNP) is a licensed advanced practice registered nurse (APRN) who functions as a member of health care team to provide high quality, comprehensive acute and/or routine mental health care of patients seen by our Family Connects team within the scope of practice granted in accordance with the RUMC medical staff bylaws. The PMHNP will promote collaboration between multiple disciplines to ensure cost effective and quality patient care. The individual who holds this position will exemplify the Rush mission, vision and values and act in accordance with the policies, procedures, guidelines of Rush. Successful candidates will also meet all other state and federal regulatory practice and accreditation standards.
